For Non-Contact Football Camp Participants & Parents/Guardians
This agreement is made between Blitz Football Ltd. (“the Camp”), the Player (participant), and the Parent/Guardian.
By signing below, you both agree to the following:

1. Participation & Conduct
We expect every player to show up with a good attitude, ready to learn, work hard, and respect coaches, staff, and fellow players. This is a team environment,  no bullying, or disrespect on or off the field.

Parents/Guardians: We ask you to support this effort by encouraging positive behaviour and sportsmanship from the sidelines.

2. Non-Contact, Not No-Risk
While this is a non-contact camp, physical activity always carries some risk. By signing this form, you agree that Blitz Football Ltd., including its coaches and staff, are not liable for any injuries, accidents, or unexpected incidents that may occur during camp hours.

3. Emergency Care
In the event of an emergency, we’ll always do our best to contact you first. But if we can’t reach you, you authorize us to provide (or secure) medical treatment for your player if needed. This includes first aid and emergency care.

4. Refund Policy
We get that plans change, but space is limited and staff are committed in advance.

Full refunds are available until June 10th
After that, no refunds will be issued (except in cases of camp cancellation due to our error)

5. Photos & Videos
We love capturing moments from camp to share with families and promote future programs. By signing this form, you give Blitz Football Ltd. permission to take and use photos or videos of your player during camp, for social media, flyers, or our website. No names or personal info will be shared without separate permission.

6. Agreement to These Terms
We believe responsibility starts with showing up, listening, and signing your name to something that matters. That’s why both the Parent/Guardian and Player are required to read and sign this agreement. It’s not just paperwork, it’s a commitment.
